Availability Change Forms
Note: In the event of a change in availability, it is the responsibility of the employee to promptly complete the availability change form and notify the Staffing department.
As per the Nurses Bargaining Agreement, an employee cannot change their availability for the summer months of June to August. Availability must match the letter of appointment.
This form must be completed by the first day of the month prior to the month in which your availability is changing.
