Menno Place is required to collect the following information from all of our Team Members:
1. Personal Health Number (PHN)
Menno Place is required by the Public Health Officer, Dr. Bonnie Henry and Fraser Health to report all team member Personal Health Numbers (PHN). You must report your PHN number for each location on the Menno Place campus where you are employed (Home, Hospital, Apartments, MBS, SimpeQ). The information collected via this form is done by employee number only (no first or last name collected) in order to ensure that we do not compromise the security of your PHN number.
2. Emergency Contact Information
We are updating our Emergency Contact information to ensure that we have current information on record for each Menno Place team member.
3. Vaccination Status – Changes for fully vaccinated staff!
Fraser Health has announced important UPCOMING changes for long-term care/assisted living staff who are fully vaccinated for COVID-19 (2 doses of vaccine + 14 days). This also includes those who work in independent living.
What does it mean to be fully vaccinated? You are 14 days past your 2nd dose of COVID-19 vaccine.
- EFFECTIVE JULY 19, 2021 at 9:00am – Discontinuation of Wearing Masks and Eye Protection for FULLY vaccinated staff: Fully vaccinated staff (2 doses + 14 days) must self-report their double dose status. Once confirmed, they will be not be required to wear a mask or eye protection unless clinically indicated for individuals on precautions; when there is a risk of contamination due to splashing of body fluids; required based on their point-of-care assessment or to protect against the normal hazards in their regular work. We are collecting vaccination status in order to ensure the staff who are double-vaccinated are able to discontinue masks and eye protection as soon as possible.
- PENDING – (Start date to be determined) – Ability to work across Multiple Healthcare Sites: Fully vaccinated staff will (soon) be permitted to work in more than one long-term care/assisted living site. The details of how the multiple sites will work has not, yet, been announced. We are collecting vaccination status in order to ensure that staff who are double-vaccinated are able to work across sites as soon as possible.

What do you need in order to complete this form?
- An image of your vaccination card showing the 2 dates you were vaccinated (.jpg, .jpeg, .png). If you do not have a card, please take a screenshot or photo of your BC Health Gateway evidence of double-dose.
- Your PHN number found on either your Driver’s License or BC Services Card.
- Emergency Contact Information: Name, Phone Number, Relationship
