Influenza Prevention Policy

As of December 1 each year, you will need to have reported your flu immunization or decision to mask to your supervisor in order to comply with the Provincial Influenza Prevention Policy.

Where can I get vaccinated?

There are a wide variety of convenient options available to you to get your flu shot, free of charge.

  • Staff Drop-In Clinics (Nov. 18 and 21)
  • Participating pharmacies
  • Family physician
  • Walk-in clinic

How do I report my flu shot?

You can report your flu shot to Menno Place Human Resources Department, providing evidence that you have had the shot. You may also use our Influenza Self-Reporting System that allows employees to report their influenza vaccination status to Menno Place through our web-based form. This method of reporting is completely secure and confidential.

Instructions

You can self-report immediately after you have received your influenza vaccination. It is best to report this as soon as you can. A notification is sent to Menno Place Human Resources, but may take several days to be made available to your supervisor as verification of immunization. You are also required to self-report if you have decided against vaccination and your choice will be to wear a mask when in a care area this flu season.

After you have submitted, you will receive notification that you filled out this form. Your supervisor may require further proof of immunization which you should have available if asked.
