Re: Paper Reports for Incident Reports are No longer Accepted as of Oct 15th

This is a reminder to all staff that that paper incident reports, near misses, or other workplace hazard reports will no longer be accepted after October 15, 2025. Moving forward, all incident reporting must be completed via QRM (Quality Risk Management system) on Surge.

Incident Reporting
In LTC
All incidents involving staff, students, visitors, or contractors must be documented in QRM. (For Resident Incidents→ report to nurse → nurses will document in PCC Risk Management). See memo: https://mennoplacestaff.com/memo-pcc-risk-management/
In AL & IL
Independent Living and Assisted Living do not have PointClickCare, therefor all incidents involving staff, students, visitors, or contractors — including Resident/Tenant Incidents — must be reported in QRM.
Near Miss Report
LTC
For staff, students, visitors, and contractors. (Resident near misses → report to nurse→ nurses will documents in PCC Risk Management.) See memo: https://mennoplacestaff.com/memo-pcc-risk-management/
AL & IL
All near misses involving staff, students, visitors, and contractors — including Resident/Tenant Incidents — must be reported in QRM.
Workplace Hazard Report
LTC
Report hazards such as clutter, tripping risks, pets, etc in QRM
AL & IL
Report hazards such as clutter, tripping risks, pets, etc in QRM
Employee Incident Investigations Report
LTC
Fill this report in QRM if you as a staff member are exposed to harm.
If resident is also involved in this incident, notify the nurse → nurse completes Resident Incident Form in PCC Risk Management. See memo that outlines the PCC process: https://mennoplacestaff.com/memo-pcc-risk-management/
AL & IL
Fill this report in QRM if you as a staff member are exposed to harm.
If resident is also involved in this incident, complete a separate incident report on QRM
