Thank you to all staff for working with us as we learn to use ComVida together. We appreciate your time, effort and patience as we migrate hundreds of staff into a new system.

Starting Sept 2, 2025 the Staffing Office will no longer call you for open shifts. All shift offers will only be sent through the ComVida app.

Menno Place is launching a new Quality and Risk Management (QRM) system within Surge Learning, starting August 27, 2025!
MEMO: PCC Risk Management – LTC Resident Incidents
As part of our collaborative effort within the clinical team, we’re improving how we document resident behaviours and incidents in long-term care. Starting August 27, 2025, all resident-related incidents in Menno Home and Menno Hospital will be documented in PointClickCare (PCC) under Risk Management by nurses only.
Why this change?
By moving all LTC resident behaviour and incident documentation into PCC Risk Management (just like Falls), we will:
- ✅ Have one consistent, centralized system
- ✅ Make tracking, reporting, and learning from incidents easier
- ✅ Strengthen resident care, safety, and compliance
What’s Changing (Effective August 27):
- Home and Hospital Staff who witness an incident → provide a written and/or verbal witness statement to the LPN on duty.
- The LPN will record the incident in PCC under the Risk Management tab using the correct category.
- Staff are no longer required to complete a separate Incident Report form – the nurse will enter all necessary details in PCC.
Categories Available in PCC Risk Management:
Along with Falls, the following categories are available:
- Verbal Aggression
- Physical Aggression
- Choking
- Elopement
- NEW: Skin Impairment
- NEW: Pressure Wounds (facility-acquired and acute care–acquired)
- Suicide Ideation
- Sexually Inappropriate Behaviour
- Near Misses (resident-exhibited)
- Other (for incidents not covered above)
How We’ll Do This Together:
- 📅 Education sessions will be scheduled for all LTC nurses in the coming days.
- 📝 Sessions will cover the new process, provide practice using PCC Risk Management, and allow time for questions.
- 💡 Your feedback is important – please share what works well and what could be improved so we can fine-tune this process together.
