Surge – QRM Announcement (For LTC, AL & IL)

Menno Place is launching a new Quality and Risk Management (QRM) system within Surge Learning, starting August 27, 2025!

MEMO: PCC Risk Management – LTC Resident Incidents

As part of our collaborative effort within the clinical team, we’re improving how we document resident behaviours and incidents in long-term care. Starting August 27, 2025, all resident-related incidents in Menno Home and Menno Hospital will be documented in PointClickCare (PCC) under Risk Management by nurses only.

Why this change?

By moving all LTC resident behaviour and incident documentation into PCC Risk Management (just like Falls), we will:

  • ✅ Have one consistent, centralized system
  • ✅ Make tracking, reporting, and learning from incidents easier
  • ✅ Strengthen resident care, safety, and compliance

What’s Changing (Effective August 27):

  • Home and Hospital Staff who witness an incident → provide a written and/or verbal witness statement to the LPN on duty.
  • The LPN will record the incident in PCC under the Risk Management tab using the correct category.
  • Staff are no longer required to complete a separate Incident Report form – the nurse will enter all necessary details in PCC.

Categories Available in PCC Risk Management:

Along with Falls, the following categories are available:

  • Verbal Aggression
  • Physical Aggression
  • Choking
  • Elopement
  • NEW: Skin Impairment
  • NEW: Pressure Wounds (facility-acquired and acute care–acquired)
  • Suicide Ideation
  • Sexually Inappropriate Behaviour
  • Near Misses (resident-exhibited)
  • Other (for incidents not covered above)

How We’ll Do This Together:

  • 📅 Education sessions will be scheduled for all LTC nurses in the coming days.
  • 📝 Sessions will cover the new process, provide practice using PCC Risk Management, and allow time for questions.
  • 💡 Your feedback is important – please share what works well and what could be improved so we can fine-tune this process together.

This is not just a process update — it’s a team effort to strengthen how we protect and care for our residents. Thank you for bringing your experience, insights, and dedication to this important step forward.

If you have questions, ideas, or need clarification, please connect with your Clinical Team Leader.

Thank you for your continued support and teamwork!

MEMO Re: Long Weekend Sick Calls – Labour Day

To: All Menno Place Staff
From: Jessy Manhas
Date: August 21, 2025
Re: Long weekend Announcement Regarding Sick Calls

Dear Team,

In the past, we have noticed a pattern of increased sick calls over the long weekend, followed by staff working on the statutory holiday the next day. All staff scheduled to work during the long weekend are expected to report to their shifts as usual.

Due to this recurring pattern, we will require proof of illness/doctor’s note as per the Collective Agreement from anyone who calls in sick on Aug 31, 2025. Additionally, if you call in sick on Aug 31 and then come to work on the statutory holiday, September 1,  an investigation will be initiated by your Manager with a Shop Steward and a doctor’s note is mandatory. Failure to comply will result in disciplinary action and a risk of no sick time pay.

We appreciate your understanding and dedication to our residents’ care. Adhering to these guidelines will help ensure smooth operations throughout the long weekend.

If you have any questions or concerns, please contact your department manager.

Sincerely,

Jessy Manhas
Manager of People & Culture
