Since you are a full-time or part-time staff member at Menno Place who interacts with residents, we are asking for your anonymous input into our Safety Culture.

You could win a gift certificate to the Greek Island restaurant! We need 200 full-time and part-time staff to complete this Accreditation Canada anonymous survey by November 10th.

If you are a FULL-TIME or PART-TIME Menno Place staff (excluding Finance, Staffing & Laundry), please fill out this survey.

Why do this survey?
Safety culture impacts the quality of care and affects staff and resident safety. This survey tool measures key safety culture components that can help inform us of our safety practices. It also provides feedback from staff on how we approach resident safety and report safety incidents.

Who administers the survey?
This survey is prepared by Accreditation Canada and goes directly to them. The survey is anonymous and is designed to provide feedback to all of us in order to improve the safe delivery of care at Menno Place.

When do I fill it out? Is there a PRIZE? (YES!)
If 200 staff fill out the survey between October 13th and November 10th, we will draw a prize winner for a gift certificate to the Greek Island restaurant! (Only full-time and part-time staff are included in this prize draw).

Who can fill it out?
Only full-time and part-time staff members who interact with residents are eligible to complete the survey. Casual staff and those who are not directly interacting with residents are asked not to participate.

Thank you for your participation. It makes a difference!
